Employee of the month form

The following guidelines are in effect for the Employee of the Month award: • All employees, except Management team are eligible for award • All employees are allowed to nominate themselves (i.e. there might be something that you have achieved for the company that resulted in great success that others might not beware of) • This is for employees who have gone above and beyond. Please provide specific examples. • The Management team will choose the Employee of the Month • The Employee of the Month reward will be announced during the monthly Company Announcements Meeting • The Employee of the Month will receive a $25 gift card
